We welcome you as a patient and appreciate the opportunity to provide you with medical services. The information that follows is designed to answer questions that many patients have. We want you to know our policies and methods of practice. If you have any additional questions, please do not hesitate to ask one of our staff members.

IMPORTANT: Due to public health concerns this season regarding H1N1 and Seasonal Influenza, we would like to direct you to this handy  guide regarding the differences between seasonal influenza, H1N1 and the common cold.

To ensure that you are scheduled enough time with your physician, please provide the reason for your visit. Physicians may have to schedule a second visit if additional problems are presented at the time of service. Any visits that are scheduled on the same day of the appointment may be considered as an urgent care visit. Please remember to present your insurance information at each visit.

 

Cancellations
Please provide a 24-hour notice if you must cancel your appointment. A $50-$150 fee (fee determined by the length of the missed appointment) may be charged to your account if a 24-hour notice is not received.  For further information, please review our cancellation/no show policy.

 

New Patient

If you are a new patient to our practice, please print out the three forms below and bring them with you to your first appointment.

  Authorization to Release Medical Records from Previous Provider (optional)

 

 

Lab Results
You will be notified of your lab and test results by telephone or mail. Results may not be available for two weeks.  If you need to request results, you may visit our patient portal.


Depending on your insurance, prior authorization may be needed for some tests and/or prescriptions. Our office will process these for you, though ultimately you are responsible for verifying when your insurance requires these to be done. Some specialists may request that you see your primary care physician before scheduling you. Our providers will not issue a referral without seeing you first.
